Frequently asked questions about Health First Foundation scholarships

Eligibility


I am eligible for multiple scholarships; may I apply for more than one?

You may apply for more than one scholarship. However, you may only receive one Health First Foundation scholarship per year.


I'm enrolled in a college or university outside northern Arizona. Am I still eligible?

For most scholarships, you are eligible if you live or work in northern Arizona. For the Kary Christensen RN Education Scholarship and the Health First Board of Directors Nursing Scholarship, you must be enrolled in a college or university located in northern Arizona.

Application

Do I need to submit proof of enrollment and the Advisor Verification Form?

Yes. You must submit both.


What is an acceptable for proof of enrollment?

Acceptable forms are: An unofficial transcript, registrar enrollment verification letter, or official acceptance letter.


Does my academic advisor need to sign the Advisor Verification Form?

Yes. Your advisor must complete the bottom section of the Advisor Verification Form. The advisor's hand signature or electronic signature is acceptable.
